A doença de Crohn é uma doença inflamatória intestinal, cuja etiologia ainda se encontra pouco esclarecida. Vários estudos referem a importância de factores biopsicossociais na evolução das doenças inflamatórias intestinais, salientando a influência dos factores psico-emocionais no desenvolvimento de surtos inflamatórios. Sendo a depressão frequentemente associada ao aparecimento e agravamento de patologias orgânicas, neste estudo pretendeu-se investigar a relação entre depressão e o desencadeamento dos surtos inflamatórios que caracterizam a doença de Crohn. O método utilizado foi o estudo de caso, tendo sido utilizadas técnicas de recolha de dados como a entrevista semi-estruturada e o teste de Rorschach. Ao longo de nove sessões, acompanhámos uma mulher de 28 anos diagnosticada com doença de Crohn, utente do Serviço de Gastrenterologia do Hospital de Santa Maria. Da vida relacional do sujeito, destacou-se um significativo número de perdas e separações que despoletaram acentuados quadros depressivos. Constatámos que essas vivências depressivas precederam e acompanharam a emergência de surtos inflamatórios. A partir de uma análise compreensiva do funcionamento do sujeito verificámos a existência de uma estrutura depressiva da personalidade. Da análise do caso, concluímos que a organização depressiva da personalidade e os estados depressivos vivenciados terão potenciado os surtos inflamatórios da doença de Crohn.
ABSTRACT: Crohn's disease is an inflammatory bowel disease, whose precise cause is still not known. Several studies report the importance of biopsychosocial factors in the development of inflammatory bowel disease, highlighting the influence of psycho-emotional factors in the development of inflammatory outbreaks. As depression is often associated with the onset and worsening of organic diseases, this study intended to investigate the relationship between depression and triggering of the inflammatory outbreaks that characterize Crohn's disease. The used method was the case study, making use of techniques such as the semi-structured interview and Rorschach test. Over nine sessions, we followed a 28 years old woman diagnosed with Crohn's disease, patient at the Gastroenterology Department of St. Maria Hospital. From the subject’s relational life, a significant number of separations and losses stood out, which triggered acute depressive states. We noted that these depressive experiences preceded and followed the emergence of inflammatory outbreaks. From a comprehensive analysis of the subject’s functioning we found the existence of a depressive personality structure. From the case analysis, we concluded that depressive personality and depressive episodes triggered inflammatory Crohn's disease outbreaks.
