Verifica-se um importante incremento duma recente patologia designada por “dor crónica”. Esta patologia afeta sobretudo faixas etárias mais elevadas da população com maior prevalência nas mulheres. Considera-se que 40% a 60% das mulheres e no mínimo 20% dos homens sofrem de dor crónica, ou tenham sofrido. Esta é geralmente incapacitante e gravosa em termos da qualidade de vida. Este agravamento é também devido à comorbidade com psicopatologias associadas a quadros generalizados de depressão e de ansiedade. É frequente a ideação suicida nestes pacientes. A dor crónica é também um problema de saúde pública: O seu tratamento é muito dispendioso e de longa duração passando por equipas multidisciplinares e está na origem de elevado número de processos por incapacidade laboral e de reforma antecipada. Nestas equipas, os psicólogos são responsáveis por avaliações psicológicas e de acompanhamento psicoterapêutico. Psicoterapias de Apoio, de orientação Psicanalítica, Cognitivo-Comportamental ou Terapias de Grupo, são comumente utilizadas. Mas hoje estão a ser desenvolvidas novas técnicas diretamente vocacionadas para estes pacientes. Face a este desenvolvimento, consideramos que os pacientes de dor cronica podem formar um grupo com características e problemáticas comuns. Caso se verifique, o identificá-las e avaliá-las pode incrementar o sucesso das psicoterapias reduzindo a sua duração e custo. Objetivo: efetuar uma revisão de literatura sobre a problemática do paciente com dor cronica, do psicólogo neste contexto, sobre os pontos-chave de psicoterapias específicas e por último estudos empíricos publicados. Numa segunda fase procederemos a um estudo empírico com pacientes com dor cronica mas com alta da psicoterapia no sentido de explicitar as mudanças resultantes e as características relevantes do processo terapêutico.
ABSTRACT: There is a significant increase from a “new” disease called "chronic pain." This disease mainly affects older age people with a higher prevalence in women. It is considered that 40% to 60% of women and at least 20% of men suffer from chronic pain or have suffered. This is often incapacitating and onerous in terms of quality of life. This increase is also due to co morbid psychopathology associated with generalized condition of depression and anxiety. Often suicidal ideation is found among these patients. Chronic pain is also a public health problem: its treatment is very expensive and long duration through multidisciplinary teams. It is also the origin of large number of processes for work disability and early retirement. In these teams, psychologists are responsible for psychological assessments and psychotherapy. Psychoanalytic, Cognitive-Behavioral Therapy or Group Therapy is commonly used. But today new techniques are being developed. Given these developments, we believe that patients with chronic pain can form a group with common characteristics and problems. In the event, to identify and evaluate them can increase the success of psychotherapy reducing its duration and cost. Objective: To perform a literature review on the issue of patient with chronic pain, the psychologist main functions in this context, the key points of specific psychotherapies and identify empirical studies already published. In the second phase we will conduct an empirical study with these patients in order to determine their changes and the relevant characteristics of the psychotherapeutic process.
