Abstract(s)
Reconhecendo a importância da gestão dos hospitais
para fazer face às dificuldades que o sistema de
saúde atravessa, e constatando que são poucos os estudos
publicados sobre estas organizações, procurámos
dar um contributo (necessariamente limitado) ao estudo
desta matéria.
Após uma breve revisão de literatura, em que apresentamos
alguns dos principais modelos, interessámonos
particularmente pelo modelo da burocracia profissional,
configuração estudada por Henri Mintzberg
(1982), nos hospitais dos EUA e que Michel Cremadez
(1992) descreve também nos hospitais franceses.
Questionámo-nos pois se, no nosso país, esta estrutura
também poderia existir.
Estudámos esta questão, através da análise de
conteúdo de entrevistas realizadas junto dos membros
do conselho de administração, das chefias médicas e
de enfermagem do centro operacional, num hospital
distrital do Sul do país, que evoluiu, em poucos anos,
de uma estrutura humanista (de hospital da Misericórdia)
para uma estrutura diferenciada. Os resultados
obtidos levam-nos a concluir que a estrutura organizacional
emergente se assemelha à burocracia profissional,
descrita por aqueles autores.
Recognising the importance of hospital management to face the serious problems western health system has to deal with and verifying that in Portugal little research have been done in this area, we tried to give a (limited) contribution to this subject. From a literature review, in which we detached the most important models, we focused ourselves on Henri Mintzberg´s (1982) professional bureaucracy, for it seems a rather cross-cultural configuration. In fact, Mintzberg described it in the USA, and Michel Cremadez (1992) showed this structure in France. We figured, it might as well emerge in a portuguese hospital. Therefore, we interviewed all administration’s board members and the medical department’s chiefs and nurse hierarchy of a small hospital in the South of Portugal which have recently evolved from an humanistic structure to a more differentiated one. We have then analysed the emergent structure. From the content analysis were able to conclude that it’s structure is very similar to Mintzberg’s professional bureaucracy configuration.
